In fact, players with a save file from the original game will be treated to a different opening sequence, determined by the path chosen in the first game. The Suffering: Ties That Bind picks up immediately after this point. The original game ended with Torque finally escaping from Carnate Island as either a wrongly accused man or a heartless killer, depending on the moral path the player chose. As the game progresses, the player is forced to make a number of different moral choices, ultimately determining Torque's guilt or innocence in the involvement of the slaughter he was convicted of. Torque ends up uncovering a twisted plot involving dark conspiracies and occult experimentations, all of which mysteriously ties into the murder of his family. While Torque is awaiting execution on Carnate Island, Abbott Penitentiary comes under siege by a host of twisted demons.
